You have two options for downloading time sheets.

1: PDF format (Recommended)

Directions:
1. Click on one of the above links.
2. Print the page (it will open in a new window)
3. Fill out time sheet as usual. Be sure to sign it and have it signed by your supervisor.
4. Allow supervisor to make a copy for records
5. Fax timesheet to 740-592-6289 or bring it to the office.
